To All Medicard Cardholders!

We are pleased to inform you that the Annual Physical Examination (APE) has been scheduled for March 19, 2025 (Wednesday), from 7:00 AM to 3:30 PM at the FEU Diliman MPR Annex Building. All eligible associates are required to undergo the scheduled APE. Kindly register through this link: https://forms.office.com/r/wGmfJpyC0L?origin=lprLink  on or before February 28, 2025.

Kindly note the following:

1.   Thumb-size stool sample to be collected at home, in the morning of the APE schedule. The container will be available at the HR office on February 17, 2025

2.     Urine samples must be collected within 15 minutes before submission to the laboratory (extended time may cause contamination).

3.     Proper labeling of the specimen bottles to avoid switching of specimens and errors in the test results.

4.     At least 8-10 hours fasting for Fasting Blood Sugar (FBS) and Total Cholesterol

5.     PAP SMEAR (for females 35 years old and above) – no sexual contact within 3-5 days before the procedure and no menstruation

6.     ECG (for those 35 years old and above) – no coffee intake and no smoking for at least 3-4 hours before the procedure

Previous chest x-ray films must be submitted for comparison for all those:

1.     who had pulmonary tuberculosis

2.     had treatment or currently having treatment for pulmonary tuberculosis.

3.     with previous chest x-ray findings of active, inactive, or stable pulmonary tuberculosis

4.     with previous chest x-ray findings of scar, fibrosis, granuloma, hilar and/or paratracheal adenopathies, or

5.     with previous chest x-ray findings of suspicious densities requiring lordotic or apicolordotic view.

Also, we wish to inform you that all cardholders who will avail of eyeglasses, as prescribed by the in-house Ophthalmologist on the day of the APE, may reimburse the amount of P4,000.00 from Medicard. Payment of the eyeglasses will be through salary deduction for 4 paydays, subject to reimbursement by Medicard.  

We need at least 50 confirmed registrations to proceed with the on-site APE.
